| If tranquilizer is necessary for
treatment or handling, I give my permission to North Cobb Animal Clinic to
administer such medication. |

| All animals entering the hospital
must have proof of up-to-date vaccinations (distemper series, rabies, kennel
cough, etc.) or they will be administered upon entry, at the owner's
expense. |

| I give authorization to do whatever
is necessary should an emergency arise. Payment is required when pet is
released. |

| Upon admission, all pets will be
checked for fleas and ticks. If necessary, they will be bathed and dipped at
that time, at the owner's expense. |

| If you wish, pets will be bathed on
the day of discharge at half price. |
| Pets are released only during
regular office hours. Monday through Friday 8am-6pm. Saturday 8am-1pm. |
